Ordering Information with the Help Card
If you are a Help Card Customer, we would be happy to help you with your purchase over the phone or by email.
When placing your order we will need to have the following information.
Your Name, Phone Number, Address, Email Address, Your Application Identification Number and the information on the item that you would like to purchase.
We will verify your information and send you a total for your item along with a charge slip to be returned signed by fax or email.
Please note orders using the help card may take up to a week to process.
